| ARE THERE PARKING FEES? Yes, each team is required to pay $60 for parking for the weekend. This check should be made payable to FC PEORIA and can be mailed with the tournament fees to FC PEORIA, PO BOX 294, MOSSVILLE, IL 61552-0294. |

| Are pets allowed at the fields? Pets are PERMITTED, but pets must be on a leash, waste picked up, and pets must not interfere with game action or spectator enjoyment. The pet owner is responsible for the actions of the pet. |

| What are the hours of registration? Registration will be Friday, September 26th - 4:00 to 10:00 PM at the Wingate by Wyndham located at 7708 N. Rt 91, Peoria, IL 61615 and Saturday, September 27th - 7:00 to 9:00 AM at the MIDWEST SPORTS COMPLEX located at 12026 N. Old Galena Rd. Mossville, IL 61552. |
| What are the directions to the Wingate by Wyndham? For a map and direct direction links, go to the "Find a Hotel" link in the Fast Find section and find the Wingate hotel. From the West - Follow Interstate 74 East to Highway 6 North (Exit 87B); Take Highway 6 North to the first exit (Exit 2); At the top of the ramp turn right and stay in the right hand lane to the second light (Highway 91); turn right onto Highway 91 north and look for the Wingate about 1/2 mile on you right. From the East - Follow Interstate 74 West to Highway 6 North (Exit 87B); Take Highway 6 North to the first exit (Exit 2); At the top of the ramp turn right and stay in the right hand lane to the second light (Highway 91); turn right onto Highway 91 north and look for the Wingate about 1/2 mile on you right. From the North - Take Highway 29 south to Highway 6 south; take Highway 6 south to the third exit (Exit 2); at the top of the ramp turn right to the first light (Highway 91); turn right onto Highway 91 north and look for the Wingate about 1/2 mile on you right. | What are the required documents I need to MAIL prior to Registration? ALL DOCUMENTS EXCEPT THE PLAYER PASSES MUST BE MAILED TO LEAH LEWIS, PO BOX 682, MACINAW, IL 61755. THE PLAYER CARDS WILL BE CHECKED AT REGISTRATION. 1. Player and coaches cards - Each player and coach must have a current/valid USYSA Player Pass. These must have a hole in them and they will be bound. 2. 2007-08 Team Roster with Add/Drop Forms (if any) with the state's or league's approval stamp clearly visible. BRING A COPY THAT WE MUST KEEP. 3. Illinois Medical Release Form (See IYSA Medical Release Form in the "Forms for Registration" section.) - Each player must have a completed IYSA Medical Release form. The form does NOT need to be notarized and copies of insurance cards do not need to be attached. BRING A COPY THAT WE MUST KEEP. 4. IYSA Sanctioned Tournament Roster (See IYSA Sanctioned Tourn Roster in th "Forms for Registration" section.) - This is required by the IYSA. The original IYSA Sanctioned Tournament Roster must be submitted at registration by Illinois and out-of-state teams. This roster MUST be signed by the coach. WE WILL KEEP THIS FORM. 5. Guest Player Permit - Illinois teams must submit an Illinois Guest Player form. Out-of-state teams must use their state guest player forms and have all necessary state approvals. WE WILL KEEP THESE FORMS. 6. Travel Permit - Non-Illinois teams must have a travel permit approved by its state association to submit at registration. WE WILL KEEP THESE FORMS. 7. Tournament Team Agreement (Form NT) (See Tournament Team Agreement in the "Forms for Registration" section.) - This form and all the required attachments indicated on the form are required ONLY FOR US CLUB TEAMS. WE WILL KEEP ALL THESE REQUIRED FORMS. |

| The application form does not look right on my screen (overlapping drop down boxes, long rows over the input fields, etc.) What is going on? First, you should let the ENTIRE form load BEFORE you scroll. It is a big form. In short, your computer does not have enough memory (RAM) available to render the form correctly. Here is the solution that will work in most cases: 1. Clear the cache (temporary files) in Internet Explorer (Usually Tools>Internet Options>Remove Temporary files 2. Shut down Internet Explorer 3. Restart the computer 4. Open JUST MS Internet Explorer. 5. Go to Web site, fill out form. In most cases, this will solve the problem.... |
